Summary: This position is responsible for performing all new client comprehensive assessments and referring these clients to any and all appropriate Sweetser services. ESSENTIAL FUNCTIONS: Completes client assessments as scheduled by the Promise Line. Responsible for all assessment documentation to include: diagnosis, assessment, mental status, level of care, progress notes, releases etc within the Electronic Health Record (EHR). Submits referrals for immediate scheduling within identified standards with designated service(s). Perform concurrent documentation.
PRODUCTIVITY: Minimum of twelve (12) hours per week
EDUCATION: Master s Degree in a related field (Psychology, Human Development, Social Work, etc)
CERTIFICATES, LICENSES, REGISTRATIONS: LCSW, Independent Licensure required Maintain a valid state driver s license.
EXPERIENCE: Experience with assessment of adults, children and residential. Persons with lived experience in the Mental Health and/or Substance Abuse system are encouraged to apply.
KNOWLEDGE AND SKILLS: Knowledge of Medications, crisis assessment skills and familiarity with community resources. Computer skills, including Electronic Health Records (EHR) Customer service skills Excellent communication skills
