Rutgers, The State University of New Jersey is seeking a Palliative Care Advanced Practice Nurse for the Department of Family Medicine and Community Health at the Rutgers Robert Wood Johnson Medical School.
Working collaboratively in the hospital with a physician under the clinical supervision of the Medical Director, the APN provides medical and palliative care evaluations and ongoing medication treatment to adult patients. The specific protocols of treatment are worked out by the APN and collaborating physician through a practice agreement. The APN provides consultation to clinical staff regarding patient care issues, and may also provide formal in-service education and training to other employees. This position participates in a collegial relationship with the physicians, nurses and other health care professionals. The APN performs in an expanded nursing role and has responsibility for meeting the needs of a determined number of patients using a primary care model. This position requires a high level of professional responsibility, medical judgement, and ability to take personal initiative while working as part of a team.
Among the key duties of this position are the following: * Participates in data collection related to the Hospitalist patients' status and needs. * Provides direct patient care within scope of practice to ensure implementation of the plan of care in collaboration with members of interdisciplinary team. * Informs and instructs patients/significant others on patient's health/illness state and plan of care. * Documents all aspects of patient care in EMR system. * Assesses the palliative care needs of patients and caregivers and develops palliative care plans that include physical, psychological, social and spiritual interventions. * Able to provide appropriate treatment for patients on comfort care/hospice. * Administers medications and parenteral therapy in accordance with established policies.
