The faculty member will serve as a member of the healthcare team for the Integrated Health Home (IHH) for the Severely Mentally Ill (SMI) population. IHH integrates medical and mental health services under one roof, for the overall care of this underserved patient population. The faculty member will work closely with the inpatient psychiatry clinical faculty member to establish continuity of care for patients. The faculty member will be an integral member of the IHH multidisciplinary team, providing direct patient care to patients with serious mental illnesses. Responsibilities may include patient counseling (by telephone and in person, including home visits), medication reconciliation, developing educational materials and in-service programs for staff, providing regular medication education groups for IHH patients, assisting with quality improvement projects or clinical initiatives within the program, participating in treatment team meetings, working collaboratively with prescribers to provide optimal pharmacotherapy recommendations, providing health care team members with responses to drug information questions, and assisting prescribers with laboratory monitoring of agents such as clozapine.
