Summary: The Sr. #Social Worker will utilize strength based therapeutic skills to promote ACO patients# self-determination and positive utilization of the healthcare system. The Sr. #Social Worker will collaborate with the patient/family, primary care provider, care team, and community providers to ensure that patients receive comprehensive, accessible and coordinated care.# The Sr. #Social Worker will be an integral member of the patient#s care team and will work closely with the patient/family, community partners and the ACO Care Team members to develop a dynamic and comprehensive plan of care. The Sr. #Social Worker will monitor adherence to the plan of care and revise as needed. The Sr. #Social Worker will supervise the Community Health Workers and the ACO Social Workers in collaboration with the Nurse Care Manager and ACO Team Lead. The Sr. #Social Worker will demonstrate expertise and leadership in coordinating his/her own management of high risk members.# EDUCATION:#### Master#s degree in Social Work or related Behavioral Health field, LICSW required EXPERIENCE:# 5-10 years of progressive responsibility in human services/behavioral health care with demonstrated experience in working with people with complex challenges. Prior supervision of a team is desired. SKILLS/ABILITIES:# Innovative, creative and collaborative style with excellence in patient engagement, critical thinking, community collaboration and organization, written and verbal communication, and working knowledge of the HHS service area.# Able to work both autonomously and as part of a team with minimal supervision. The Sr. Social Worker must demonstrate leadership abilities in the daily management of a team and the ability to multi-task in a rapidly changing healthcare environment. #Proficiency with technology and business software. Reliable means of transportation for service delivery in the community.#
