The ED Nurse Care Manager will work to improve transitions of care and ensure care is being delivered in the most appropriate setting for patients who have a Melrose Wakefield Healthcare PCP.# The ED Nurse Care Manager will work closely with the#Melrose Wakefield# Healthcare#Care Management Team, Hospital Staff, Patient Ping, PCP#s, and The MCO Partner (Fallon Health) to monitor utilization of services, identify medical and social drivers of ED utilization, work to improve the quality of care by reviewing clinical information and recommending the most appropriate care setting.# The ED Nurse Care Manager will develop timely appropriate interventions for the medically complex high risk population, responsible for improving and implementing meeting their medical and psychosocial needs while avoiding unnecessary utilization and readmissions.# This position will be responsible for improving and implementing complex care management care programs for individuals with chronic medical conditions, limited functional status and psychosocial needs that are seen in the emergency department.# The ED Nurse Care Manager will work across the continuum helping patients and families to understand health care options, create and promote adherence to a care plan, and increase the patient#s ability for self-management and shared decision making and connect patients to the appropriate community resources.# The ED Nurse Care Manager will assist the Care Management Team with enrolling high risk members in Care Management program as indicated.# The ED Nurse Care Manager will promote improvements of quality of care, promote cost effective medical outcomes, provide for continuity of care, assure appropriate levels of care are received by patients, ensure appropriate transitions of care, ensure post discharge follow up by ACO and#Community Partner care teams or PHO Care Management Team.#
